WHO WE ARE

Established in 2013, Heart to Heart is an Adult Day Care Center with two easily accessible sites in Brooklyn and the Bronx. Seniors and retired people who cherish their freedom, friendships, healthy lifestyle, and medical care call Heart to Heart home. With our staff fluent in English, Spanish, and Creole, we can communicate with all our members. Our goal is to make aging more enjoyable by offering opportunities for an engaged, active, and meaningful life. This benefits our members, their families, our staff, and the greater community

HOT AND NUTRITIOUS MEALS

Every day we serve a gourmet breakfast, a delicious snack, and a fresh hot lunch! Our delicious lunch also comes with alternative meal options every day. We have a nutritionist on staff to ensure that all meals are nutritious as well as delicious! A full coffee and tea station is available throughout program hours. Come and join us for a tasty meal!

DOOR TO DOOR TRANSPORTATION

Heart to Heart provides door-to-door transportation in our brand new fleet of shuttles. With regular maintenance and enhancements on our busses, you can relax knowing that your family members are riding in the safest of vehicles.

TELECARE AND MEALS

Since the start of Corona, it's become more difficult to get out. Therefore, we provide an option to receive telecare and meal or grocery deliveries! Telehealth provide emotional support and monitors the members well being. Our Adult Day Care Center also provides complimentary meal deliveries for our members. By catering to the personal needs of each and everyone here, we make Heart to Heart feel like home.

Eligibility and Medicaid Managed Long Term Care Plans

Heart to Heart has built a stellar reputation over the last nine years as one of the most elite adult day centers in all of New York. We proudly have partnered with the following Medicaid Long Term Care Plans to assist your family. We understand the challenges of visiting a program and being told that your MLTC plan is not accepted. Instead of switching plans, we strongly encourage you to visit Heart to Heart to avoid the hassle of changing the insurance and care managers that you have grown accustomed to.

My mother with dementia attended Heart to Heart for over four years. Heart to Heart is what allowed me to live my life with the peace of mind that my mother is taken care of during the day. I know she is dancing from when I drop her off until I pick her up.
